Advanced Practice Professional (APP)
The Advanced Practice Professional (APP) has coordination and clinical skills required for the comprehensive management of critical care medicine patients, including biological, social, and psychological assessments. They will possess specialized skills in the care of patients and families undergoing operative and other invasive procedures. The APP coordinates the involvement of medical/surgical specialty teams necessary to develop a comprehensive plan for the patient and reviews the care plan with the other members of the clinical team, participates in directing that care.
The provider will work with the team to ensure adherence to protocol requirements and regulatory guidelines. They will perform diagnostic/therapeutic procedures, peri-procedural services, and coordinates discharge planning to next level of care (as applicable). The APP will participate in the education of residents and medical/nursing students. This position will also be responsible for responding to rapid response alerts. In this capacity, the APP would assess and initiate care for the critically ill patient in the activation.
Core duties and responsibilities include managing comprehensive and individualized care for patients and families in the inpatient/critical care phase of hospitalization and immediate post transfer to stepdown, ward or discharge. The APP will complete daily rounds; consults and/or pre-procedure/evaluations, complete integrated assessments, order additional tests or consults as deemed appropriate, perform and communicate results of diagnostic tests, consult and communicate with clinic and hospital staff regarding abnormal findings in the evaluations of patients, perform succinct comprehensive clinical evaluations and document findings in medical record, facilitate patient discharge or transfer by coordinating appropriate services and tests for follow-up (as applicable), develop a comprehensive individualized plan of care for critical care patients, demonstrate expertise in caring for patients across the age continuum of their educational experience and certification, identify unstable or complex critical care patient care situations and facilitate appropriate intervention in the hospital setting, recognize emergency situations and initiate effective emergency care, evaluate critical care patients and serve as a coordinator for follow up procedures and tests in the ambulatory setting (as applicable), assist in the collection of outcome measures (as applicable), demonstrate critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation, and management of the individual needs of patients/families, demonstrate the ability to utilize equipment in a safe manner, base decisions made/actions taken on the relevant scientific principles, established standards of care and/or research findings, utilize teaching/learning theory and appropriate teaching strategies in the provision of education to nursing staff, hospital staff, students, patients, families and the community, promote an environment that facilitates learning by responding to appropriate patient/family preferences, priorities, and readiness to learn, update own knowledge base throughout the year, complete all identified clinical competencies, serve as a resource person for staff caring for the critical care patient, provide educational programs to professional and paraprofessional personnel in the area of expertise, participate in community programs, distribution of literature, as appropriate in area of expertise, document the plan of care, interventions, and evaluations in a clear and concise manner for every patient visit in the patient’s medical record following approved hospital/UHA policies and compliance standards for documentation, collaborate with other leaders and staff to effectively achieve patient outcome-based improvements in care, provide leadership in the development and implantation of changes in the critical care practice that positively impact patient outcomes, work collaboratively with medical and nursing staff to facilitate creative problem solving and provide professional support, participate and initiate inter-disciplinary approaches to patient care, program development and education.
Serves as a professional role model, promote and practice innovation of the expanded role in the delivery of care to critical care patients and families throughout the inpatient hospitalization, and the ambulatory setting, participate and provide leadership for committees and task forces, document hours and submit billing reports, as appropriate, review and interpret critical care data from sources such as patient registries, clinical and financial reports, length of stay, access to care, and cost per case reports and recommend/implement changes as necessary, participate in clinical trials and research studies with the Principal Investigator, assist critical care Faculty,…
