About The Role
We are seeking a Junior Clinician / Case Manager to support a high-impact healthcare engagement. In this role, you will work alongside multidisciplinary clinical teams to coordinate patient care, drive effective care transitions, and ensure appropriate utilization of healthcare resources.
Key Responsibilities
- Plan and coordinate patient care activities in collaboration with physicians, nurses, and multidisciplinary care teams
- Conduct clinical assessments and actively support care planning and implementation
- Ensure patients receive the appropriate level of care throughout their healthcare journey
- Perform timely and effective discharge planning to support smooth post-discharge transitions
- Collaborate with third-party payers to verify and ensure coverage for patient services
- Maintain accurate and compliant documentation in accordance with regulatory standards
Requirements
Required Qualifications
- Active RN or licensed clinical professional (LPN, LCSW, or equivalent)
- 1-3 years of experience in case management, utilization review, or care coordination
- Familiarity with discharge planning processes and post-acute care settings
- Experience working with managed care or third-party payer requirements
- Strong communication and organizational skills
Preferred Qualifications
- Experience in a hospital or health system environment
- CCM (Certified Case Manager) certification is a plus
- Knowledge of InterQual or Milliman criteria
Travel Requirement:No regular travel is expected. Occasional onsite presence for audits or key meetings; pre-approved with expenses covered.
