Job Overview:
Shift:
Monday to Friday 6 hours per day 9am to 3pm.
We are a community-based day program. We are seeking dedicated individuals to join our team as Direct Support Professionals/Coaches. As a coach, you will provide essential support to individuals with developmental disabilities while out in the community.
Duties:
– Assist individuals through guidance, support and coaching
– Ensure the safety and well-being of individuals under your care
– Implement ISP goals
– Maintain accurate documentation
– Advocate for individual’s rights and promote independence
– Provide direct coaching services as outlined in the Individual Placement Plan (IPP) including one-on-one training of specific skills
– Work closely with Program Supervisor to monitor Individual Services Plans.
– Work on personalized goals as per IPP.
– Work with the Program Supervisor to coordinate assessments, arranging for the development of individualized behavior support plans as needed.
Requirements:
– High School Diploma/GED
– Being able to work out in the community
– Must have a valid Drivers license and a good driving record
– Must pass DOJ/FBI background check
– Clear a TB test
