About The Role

We are seeking a dedicated and professional Pharmacy Care Coordinator to join our remote team. This role involves communicating effectively with physician offices to manage prescription refills, clarify requests, and ensure accurate documentation within our systems. The ideal candidate will possess strong communication skills, attention to detail, and the ability to work independently in a fast-paced environment. The Pharmacy Care Coordinator will play a crucial role in supporting patient care by facilitating seamless pharmacy communication, resolving inquiries, and assisting with drug therapy issues. Qualifications

  • Active Pharmacy Technician registration with the State Board of Pharmacy
  • Bachelor’s degree in business administration, accountancy, sales, marketing, computer sciences, or related fields, or equivalent experience
  • Minimum of three (3) years of experience in a healthcare-related call center, pharmacy, physician’s office, or social service environment
  • Proficiency in MS Office applications such as Outlook, Word, Excel, and Access
  • Excellent oral and written communication skills
  • Strong organizational and time management skills
  • Self-motivated, proactive, with a professional demeanor and positive attitude
  • Ability to work independently and handle multiple priorities effectively
  • Understanding of disease states and patient care challenges is preferred

Responsibilities

  • Maintain professional communication with physician offices through oral and written channels at all times
  • Document clarification requests, follow-up actions, and progress accurately within Pathpoint Rx
  • Respond promptly to internal inquiries, ensuring resolution within established timeframes
  • Make outbound calls to physician offices to request prescription refills and update on pending clarification requests
  • Verify completeness and accuracy of written prescription clarifications and refill requests
  • Adhere to all relevant policies, procedures, safety rules, and regulations
  • Assist patients and Pharmacy Care Coordinators with drug therapy and pharmacy care issues, understanding the physical, psychological, and social challenges of specific disease states
  • Perform additional duties as assigned to support team objectives and organizational goals
