Position Overview

We are seeking a dynamic, organized, and highly skilled Director of Nursing (DON) to oversee all clinical operations for our home health agency. You will lead our field staff, ensure strict compliance with state and federal regulations, manage quality assurance initiatives, and champion the highest standards of clinical excellence.

Key Responsibilities

• Clinical Leadership: Supervise, mentor, and evaluate the clinical field team (RNs, LPNs, HHAs, and therapists).
• Regulatory Compliance: Ensure agency alignment with Medicare Conditions of Participation (CoPs), state licensing regulations, and federal guidelines.
• Quality Assurance & Performance Improvement (QAPI): Oversee the QAPI program, conduct regular chart audits, and implement corrective action plans as needed.
• Patient Care Coordination: Review and approve plans of care, oversee OASIS documentation, and manage patient intake and case assignments.
• Staff Development: Lead orientation, ongoing clinical training, and competency assessments for clinical staff.
• Collaboration: Work closely with the Administrator, medical directors, physicians, and families to optimize patient care and resolve clinical issues.

Qualifications & Requirements

• Licensure: Active, unencumbered Registered Nurse (RN) license in the State of Ohio.
• Education: BSN preferred, or equivalent comprehensive clinical leadership experience.
• Experience: * Minimum of 2–3 years of clinical nursing experience in Home Health care (required).
  • At least 1–2 years in a supervisory or management role (e.g., Clinical Supervisor, DON, or Assistant DON).
• Skills: * Deep understanding of Medicare guidelines, OASIS documentation, and state home health regulations.
  • Excellent communication, interpersonal, and team-building skills.
  • Proficiency with Electronic Medical Records (EMR) and home health software.
• Background: Must pass a comprehensive background check and drug screening.
