YOUR ROLE
You’ll serve as a dedicated guide for Medicare families navigating dementia care – providing personalized, in-home support backed by Localposh’s coordinated care systems. Provide in-home care coordination and concierge support for dementia-affected families
Maintain compassionate communication with family caregivers

WHO WE’RE LOOKING FOR
Experience in dementia care, home health, or elder care coordination Compassionate, professional, and highly reliable Comfortable working independently in a client’s home Strong communication skills with families and care teams CNA, HHA, PCA, EMT, Social Work, or related certification a plus
