Join our dedicated healthcare team as a Home Health Aide working from 9 a.m. to 1 p.m., where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. You will play a vital role in promoting independence, safety, and comfort through attentive patient observation and assistance with daily activities. Duties
- Monitor patients’ health by observing vital signs, medication adherence, and overall well-being, ensuring safety at all times.
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary.
- Support clients with meal preparation and nutritional needs, including cooking and feeding assistance tailored to dietary restrictions.
- Maintain accurate medical documentation within Electronic Health Record (EHR) systems while respecting patient confidentiality under HIPAA regulations.
- Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care strategies and behavioral support.
- Perform light housekeeping duties such as cleaning, laundry, and maintaining a safe environment to promote overall health and comfort.
- Collaborate with healthcare professionals by updating medical records and communicating patient status effectively during home & community care visits.
Qualifications
- Prior experience in home health care, assisted living, nursing homes, or long-term care environments is highly preferred.
- Knowledge of medical terminology, medication administration, catheter care, hospice support, and CPR certification are essential.
- Experience working with individuals with developmental disabilities or dementia enhances your ability to provide compassionate specialized care.
- Ability to operate EMR/EHR systems efficiently for accurate medical documentation and record keeping.
- Strong patient observation skills combined with the ability to assist with ADLs confidently in a home setting.
- Valid certification in CPR; additional certifications such as Hoyer lift operation or hospice care are advantageous.
- Demonstrated commitment to patient privacy and confidentiality in accordance with HIPAA standards.
