Job Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our dynamic healthcare team. The ideal candidate will demonstrate a strong commitment to delivering high-quality, patient-centered care in a variety of clinical settings, including long-term care, assisted living, and acute care environments. Under the supervision of registered nurses and physicians, the LPN will play a key role in supporting patient wellness and treatment outcomes.
Available Shifts:
- Available Shifts:
- First Shift: 6:30 AM – 3:00 PM
- Second Shift: 2:30 PM – 11:00 PM
- Third Shift: 10:30 PM – 7:00 AM
Key Responsibilities:
- Administer medications and treatments as prescribed by licensed healthcare providers.
- Perform basic patient assessments, including vital signs and routine physical evaluations.
- Assist in the creation and execution of personalized care plans.
- Provide support with Activities of Daily Living (ADLs) in long-term care, hospital, and outpatient settings.
- Monitor patient conditions and promptly report any changes to the supervising RN or physician.
- Document patient information accurately and in compliance with HIPAA and organizational standards.
- Collaborate with nurses, physicians, therapists, and social workers to deliver coordinated care.
- Perform or assist with IV infusions, catheterizations, tube feedings, and wound care.
- Educate patients and families on treatment plans, medications, and health maintenance.
- Support discharge planning and participate in case management processes.
Preferred Experience Areas:

- Emergency Medicine / Hospital Medicine
- Home Health / Hospice Care
- Developmental Disabilities / Dementia Care
- Geriatrics, Pediatrics, or Behavioral Health
Skills & Qualifications:
- Active LPN license in good standing.
- Experience with EMR/EHR platforms such as (Point Click Care (PCC)) preferred.
- Working knowledge of medical terminology, ICD-9/ICD-10, and CPT coding.
- BLS certification (Basic Life Support) required.
- Excellent communication and time-management skills.
- Compassionate and empathetic approach to patient care.
