Role Description This is a full-time remote role for a Medical Scribe at Urban Alchemy. The Medical Scribe will accurately document patient encounters in real time, including histories, assessments, treatment plans, and follow-up recommendations, while working closely with healthcare professionals. Daily tasks include reviewing and updating electronic health records, ensuring proper use of medical terminology, and maintaining clear, organized documentation that supports high-quality care. The role also involves coordinating with clinical staff to clarify information, tracking key health indicators, and assisting with documentation related to mental health, substance use, and social determinants of health. The Medical Scribe will follow strict confidentiality standards, adhere to organizational protocols, and support UA’s mission of respectful, person-centered service.
Qualifications

  • Candidates should possess strong scribe skills, including accurate real-time documentation and familiarity with electronic health record systems.
  • Candidates should possess a solid understanding of medical terminology and human anatomy to ensure precise and clinically relevant documentation.
  • Candidates should possess foundational medical assisting skills, such as organizing clinical information and supporting care workflows.
  • Candidates should possess clear, professional communication skills for collaborating with healthcare providers and engaging respectfully with diverse populations.
  • Additional beneficial qualifications include experience in behavioral health or community health settings, knowledge of HIPAA and patient confidentiality standards, and strong attention to detail and time management.
  • Relevant education such as a certificate or coursework in medical scribing, medical assisting, or a health-related field is preferred; prior experience as a medical scribe or in a clinical support role is an advantage.
