Nutrition Specialist

Location: Trillium Place

Compensation: $25.00 to $28.00 / hourly

Current Opening:

• Full-time | 8:00 AM – 4:30 PM

The Nutrition Specialist supports the nutritional care and hydration needs of residents to promote optimal health and well-being. Responsibilities include conducting resident assessments, maintaining accurate and timely documentation, contributing to the care planning process, and collaborating with the Registered Dietitian and Dining Services team to ensure quality resident care and service. This position will be performed in accordance with TANDEM LIVING Core Values, Mission Statement, and regulations.

QUALIFICATIONS:

• High school diploma or equivalent required.

• Maintain a current ServSafe Certification.

• Maintain active certification as a Certified Dietary Manager (CDM) and membership in the Dietary Managers Association (DMA).

• Demonstrate strong assessment skills and possess a minimum of two (2) years of charting experience.

RESPONSIBILITIES INCLUDE:

• Complete and maintain resident nutrition-related documentation, including:

  • Preference sheets within 48 hours of admission
  • Care plans
  • Progress notes and charting

• Review, verify, and monitor resident diets upon admission and throughout their stay to ensure accuracy and appropriateness.

• Promote and monitor adequate nutrition and hydration for residents in accordance with applicable regulatory standards, including DHS and DOH requirements.

• Provide alternative menu selections that align with resident preferences, dietary restrictions, nutritional needs, and cost considerations.

• Assist residents with meal selections and meal setup as needed to support a positive dining experience.

• Participate in care plan meetings, interdisciplinary team conferences, and other resident-centered discussions to support quality care outcomes.
