Role Description

This is a full-time remote role for a Family Support Coordinator. The Family Support Coordinator will work directly with families to provide assistance, manage cases, and connect families to relevant resources and services. Core responsibilities include collaborating with families to develop individualized support goals, coordinating services, and maintaining detailed records of case progress. Effective communication with families, service providers, and stakeholders will be a key aspect of this role.

Responsibilities

  • Serve as a primary point of contact for enrolled families via phone, email, and virtual meetings
  • Provide ongoing support and guidance to families participating in early intervention services
  • Assist families with onboarding, intake documentation, and program orientation
  • Coordinate appointments, evaluations, meetings, and follow-up communications
  • Maintain accurate client records and update case notes in internal systems
  • Communicate with service providers, therapists, and internal team members regarding scheduling and family needs
  • Help families access community resources and educational materials
  • Monitor family engagement and follow up with families as needed
  • Respond to inquiries in a professional and timely manner
  • Support administrative tasks related to program operations and service coordination
  • Participate in virtual team meetings and training sessions
  • Maintain confidentiality and professionalism in handling sensitive family information

Qualifications

  • Strong background in Human Services, Social Work, and Family Services
  • Proficiency in Case Management and developing individualized support plans
  • Excellent Communication skills to effectively collaborate with families and stakeholders
  • Exceptional organizational skills and attention to detail
  • Ability to work independently in a remote setting
  • Experience working in a related field or directly with families is preferred
  • Bachelor's degree in Social Work, Human Services, or a related discipline is highly desirable
