Evolve Health Medicine is a functional and integrative wellness practice in Port Jefferson, NY, founded by Dr. Erika Jurasits, DO, MPH, IFMCP. With more than 20 years of medical experience, Dr. Erika began her career in family medicine before recognizing that true healing required a deeper approach. Her advanced training and certification with the Institute for Functional Medicine (IFMCP) gave her the tools to help clients not only get well, but stay well – by identifying and addressing the root causes of illness.
That vision led her to create Evolve Health Medicine – a place where clients are supported through a root-cause, whole-person approach that restores balance and empowers each individual to take control of their health. Dr. Erika specializes in guiding people through the five pillars of health: hormones, detoxification, nutrition, fitness, and relaxation, so they can experience lasting transformation.

3. ABOUT THIS POSITION
The Wellness Assistant is the operational heartbeat of Evolve Health Medicine – the person who makes sure every member visit runs smoothly, every clinical detail is in place before the physician walks in, and nothing falls through the cracks after the visit ends. This is not a passive support role. It calls for clinical curiosity, financial literacy, strong systems thinking, and a genuine investment in functional health.
In your first 90 days, you will make an immediate impact where it matters most – removing friction from the physician’s daily workflow so that Dr. Jurasits can do what she does best. That means having every appointment prepped and documented in Cerbo before the provider walks in, getting visit note templates built and in active use so notes are completed in real-time with no backlog, and ensuring every member leaves with their visit summary and care plan in hand. It also means getting the practice’s AR into a clean, well-maintained state – and building the system that keeps it that way. The SOPs and processes you develop along the way will become the foundation this role runs on, and you will be fully supported by the team throughout.

4. KEY RESPONSIBILITIES
Member Care & Clinical Coordination
- Prepare all paperwork, intakes, and lab documentation for each member visit, ensuring the physician has everything needed before entering the room
- Coordinate with external labs (e.g. LabCorp) to resolve outstanding orders, missing results, and insurance coding requirements (ICD-10)
- Set up appointments in Cerbo, entering relevant clinical notes and pertinent member history to support the physician
- Check members out at the end of each visit – processing supplements, scheduling follow-ups, distributing handouts, and collecting payment
- Monitor the member portal and texting app queue, routing requests to the appropriate team member and handling administrative inquiries directly
Revenue Tracking & Financial Administration
- Own daily revenue tracking and input into the practice’s Daily Action Plan (marketing stats provided by the Enrollment Coordinator)
- Conduct monthly Accounts Receivable monitoring and deconstruction
- Take payment on all charges and maintain accurate financial records
- Support member transitions into the Longevity VIP membership program to contribute to practice growth
Member Calls & Outreach
- Conduct Awesome Calls as part of the team’s collective effort to celebrate and support members (sporadic, shared across the team)
- Participate in Qualifying Calls alongside the Enrollment Coordinator twice per month
- Support Discovery Calls as needed and develop familiarity with the full call process over time
Operations & Systems
- Answer phones, manage emails and incoming mail, and maintain office supply inventory
- Handle morning and evening duties – setting up and closing down the office each day
- Create and maintain SOPs and Routing Forms for all responsibilities within this role, in collaboration with the physician and team
- Attend all team huddles, weekly 1:1 with Dr. Jurasits, and project meetings as required
- Attend company quarterly meetings (minimum 2 per year; at least 1 must be attended in person), with travel typically spanning Wednesday through Sunday
Seminars, Webinars & Events
- Attend all in-house and dinner seminars (typically twice a month on Tuesdays until 8:30 PM) and actively support the Enrollment Coordinator with setup, enrollment, member greeting, seating, and the close
- Provide webinar moderation support on Mondays when needed (approximately 1–2 times per month, until approximately 7:00 PM)
- Complete the Evolve Health Medicine 60-day detox program after the 3-month probationary period — so you can speak to the client experience from the inside
5. SKILLS & QUALIFICATIONS
Required
- Google Workspace, Dropbox, Excel, Word & Adobe (Day 1)
- Lab portal experience and ability to interpret and code lab work for insurance (ICD-10)
- Financial literacy – revenue tracking, AR management, and Excel proficiency
- Exceptional organization and attention to detail – multiple priorities, zero dropped balls
- Strong verbal and written communication – warm, clear, and professional
- Comfort with payment conversations and collecting fees from members
- Ability to create systems, follow through independently, and thrive in a fast-paced team
Preferred (Nice to Have)
- Clinical background (MA, LPN, or equivalent) – a significant advantage for the clinical coordination component
- Experience in functional medicine, integrative health, or a wellness-focused clinical setting
- Familiarity with EHR systems (Cerbo experience a plus)
- Experience with Canva, Loom, Go High Level, Demio, or Mighty Networks

7. COMPENSATION & BENEFITS
Salary: $48,000–$52,000 per year, based on experience
Bonus: $300–$1,500 per month, revenue-triggered

Schedule:
- Monday: 7:45 AM – 6:00 PM
- Tuesday: 9:00 AM – 6:00 PM
- Wednesday: 9:00 AM – 6:00 PM
- Thursday: 7:45 AM – 6:00 PM
Please note: At least 2 Tuesdays per month extend until approximately 8:45 PM for in-person seminars. Mondays may extend until 7:00 PM for webinar coverage 1–2 times per month. On weeks with evening commitments, earlier departure on Wednesday or Thursday is available where possible. All hours are subject to change slightly.
