Continuum of Care Coordinator

What You Will Do:

• Provide coordination of patient care throughout the healthcare continuum.
• Provide coordination of communications with patients, families, physicians, and healthcare team members.
• Coordinate patient scheduling with appropriate resources or clinical services and maintains required documentation.
• Support standard patient access initiatives across the healthcare continuum.
• Provide support with navigating the healthcare system and assists patients with utilizing available community support services.

What You Will Need:

• High School diploma equivalency with 2 years of cumulative experience OR Associate’s degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Additional Preferences:

• Rotating On-Call Schedule: Sat and Sun 8am-5pm – in rotation with staff
