Job Summary

Duties

  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
  • Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to track health status accurately
  • Support medication administration following prescribed instructions while maintaining compliance with HIPAA regulations and medical documentation standards
  • Provide companionship and emotional support to promote mental well-being, especially for clients with dementia or Alzheimer’s disease
  • Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe home environment
  • Use Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities precisely and maintain medical records securely
  • Assist with specialized care needs including catheter care, hospice support, memory care strategies, and working with individuals with developmental disabilities or disabilities

Requirements

  • Proven experience in home health care, nursing homes, assisted living facilities or long-term care environments
  • Knowledge of medical terminology and familiarity with medical documentation practices
  • Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice care are a plus
  • Ability to work independently in patients’ homes while adhering to safety protocols and HIPAA privacy standards
  • Strong patient observation skills to detect changes in health status promptly
  • Experience working with individuals with disabilities or cognitive impairments like dementia or Alzheimer’s disease is highly desirable
  • Physical ability to assist with patient mobility and perform light housekeeping tasks effectively
